ADVANCE HEALTH CARE DIRECTIVE (AHCD)
An advance health care directive is a written legal document that gives the authority and right to someone else to act on your behalf regarding medical decisions for you in the event that you are unable to do so for yourself. You can include instructions regarding your future medical care, such as life-support treatment. This document should be provided to your healthcare provider. The powers granted in your advance health care directive can be revoked or changed by you as long as you are competent to do so. These powers will end when you die.
